Step 1: Fill in the online claims form or download the form and submit.

Step 2: Complete the Client Identification form first.

Step 3: Choose claim type to make (either Loan/partial withdrawal etc options)

Step 4: If you choose online form, go ahead and submit the form. If you choose download form, kindly complete the form and submit to claims@siclife.com.gh
